Here is David Leonhardt’s list.  Here is David Leonhardt’s very good column on why fee-for-service is such a fundamental problem behind American health care institutions.

“Hospitals that don’t use the fee-for-service model, like those run by the Veterans Health Administration, are already getting better results for less money.”

I don’t know anyone who really knows anything about health care who believes that the VA is a better healthcare system.

They provide limited services to a limited population and generally provide services only to those with proximity to care (VA facilities are built largely based on Congressional politics).

Most of the discussion on healthcare reform comes from those who know very little about healthcare.
